AI Decision Log Template: Turn a ChatGPT Conversation Into a Decision Record
A copyable AI decision log for turning a useful ChatGPT conversation into a decision record with inputs, options, evidence, assumptions, and next steps.
May 13, 2026

AI prompts can lead to real decisions: a product direction, a customer reply, a pricing change, a technical approach, a research conclusion, or a campaign angle. The problem is that the decision often stays buried inside a long chat.
Highlight Reel
Preserve the decision, not the whole chat
Highlight Reel helps you select the useful turns from a long AI conversation and turn them into a clean decision handoff.
Use this AI decision log template when a ChatGPT conversation mattered enough that someone may later ask, "Why did we choose this?"
Quick Answer
A prompt decision log should capture:
- The original question or prompt.
- The options considered.
- The evidence or source material used.
- The assumptions the AI or team made.
- The final decision.
- The owner and next action.
- A link to the cleaned conversation or source pack, if useful.
Do not treat a raw ChatGPT shared link or account export as the decision log. Those can preserve context, but the decision log is the reviewed version that a teammate can scan quickly.

## Prompt Decision Log Checklist
### Before saving
- [ ] The final decision is stated in one paragraph.
- [ ] The original prompt or question is included.
- [ ] The important context is summarized, not pasted in full.
- [ ] Options are listed with tradeoffs.
- [ ] Evidence is separated from opinion.
- [ ] AI suggestions rejected by the human reviewer are recorded.
- [ ] Assumptions are visible.
- [ ] Next action has an owner.
### Before sharing
- [ ] Private prompt details were removed if they are not needed.
- [ ] Shared links do not expose unrelated conversation turns.
- [ ] Source links are safe to share with the intended audience.
- [ ] The log can stand alone without the raw AI chat. Why A Prompt Log Is Not Enough
A prompt log says what you asked. A decision log says what changed because of the answer.
That difference matters when:
- multiple options looked plausible
- a teammate needs to understand the choice without reading the full chat
- a project has to preserve reasoning for later
- the AI answer mixed useful analysis with unsupported claims
- you need to reuse the decision in another tool, agent, doc, or ticket
The decision log is not an archive of every turn. It is the smallest useful record of how a prompt became a choice.
When To Use This Template
Use the full template for:
- product decisions
- technical direction
- customer messaging
- vendor comparisons
- hiring or process changes
- policy interpretation that still needs human review
- research conclusions
- agent instructions that may be reused
Use a shorter version for everyday work:

Link:ChatGPT Projects, Shared Links, And Exports
OpenAI's ChatGPT Projects documentation describes projects as spaces for chats, files, and instructions, with project memory behavior depending on project and account settings. That can help keep work organized, but it does not automatically turn a chat into a decision record.
Shared links can create a unique URL for a ChatGPT conversation. OpenAI's shared links FAQ also notes that anyone with access to a shared link can view the linked conversation and that granular permissions are not currently available. That is why a cleaned decision log is often safer and easier to read than sending the whole shared conversation.
OpenAI's export help explains how users can export ChatGPT history and account data for eligible accounts. That is useful for backup or retrieval. It is usually too broad for a teammate who only needs one decision.
Decision Log Shapes By Use Case
| Use case | Keep | Remove |
|---|---|---|
| Product direction | options, user evidence, chosen direction, tradeoffs | brainstorming dead ends |
| Engineering decision | constraints, alternatives, accepted approach, test plan | speculative code paths not used |
| Customer reply | final answer, source policy, tone choice | private internal debate |
| Research conclusion | source table, confidence, caveats, recommendation | unverified AI filler |
| Team process | reason for change, owner, review date | unrelated complaints |
Fictional Filled Example
This is a fictional placeholder example, not a real client story, customer metric, or compliance record.
# Prompt to Decision Log
## Decision title
Use a clean handoff link instead of a raw shared ChatGPT link for client-facing research.
## Date
2026-05-12
## Owner
Research lead
## Original prompt or question
What is the safest way to share an AI-assisted competitor research conversation with a client?
## Context given to the AI
- The conversation includes public sources, internal notes, and early pricing ideas.
- The client only needs the conclusion and sources.
- The team wants a reusable format for future research handoffs.
## Options considered
| Option | Why it was considered | Tradeoff | Evidence |
| --- | --- | --- | --- |
| Raw shared ChatGPT link | Fastest way to share the conversation | Exposes unrelated turns and has link-based access limits | OpenAI shared links FAQ |
| Data export | Preserves account history | Too broad for one client handoff | OpenAI export help |
| Clean decision handoff | Shows conclusion, evidence, caveats, and next action | Requires a review pass | Internal workflow need |
## Human review
- Accepted: use a clean handoff link.
- Changed: remove internal pricing notes.
- Rejected: sending the entire raw chat.
## Final decision
Send a clean handoff link with the short answer, source table, caveats, and next step. Keep the raw AI chat internal.
## Assumptions and risks
- Assumption: the client does not need to continue the original AI conversation.
- Risk: the handoff may omit useful nuance if the review pass is too aggressive.
- What would change this decision: the client requests the full source workspace and all content is safe to share.
## Next action
- Owner: research lead
- Due: before client email
- Destination: client-ready handoff pageHow Highlight Reel Fits
Highlight Reel is useful when the decision is trapped inside a long AI conversation. You can select the turns that explain the prompt, options, evidence, and final choice, then package them as a clean page.
That page can sit next to the decision log:
- the log states the decision
- the clean transcript shows the supporting AI work
- the source pack keeps links and caveats visible
The goal is not to make AI work look more formal than it was. The goal is to make the choice reviewable.

FAQ
Should I include the exact prompt?
Include the exact prompt when it helps explain the decision and does not expose private strategy, secrets, or unnecessary personal context. Otherwise, summarize the prompt accurately.
Should I link to the full ChatGPT conversation?
Only link to the full conversation if the whole thread is safe and useful for the intended audience. If not, link to a cleaned transcript or decision handoff.
Is this the same as an audit log?
No. This is a practical decision record. It can support a review trail, but it is not a compliance system and should not replace required records for regulated work.
